To make a cash gift by check, print and complete the Donation Form and send it, with your check payable to the American Mathematical Society.

Gifts by Credit Card On-Line, Phone or Fax
You can charge your credit card by using the AMS On-line Donation Form, by printing the Gift Form and filling in the appropriate information and mailing or faxing it to the AMS, or by calling the AMS. You may provide your credit card information to the AMS at (toll free) 1-800-321-4AMS (4267) in the U.S. and Canada or 401-455-4000 worldwide. You may mail or fax the AMS your donation using this simple Donation Form. The fax number is (401) 331-3842.

Transfers should include the name of the donor and the gift purpose (for example, Annual Appeal- Epsilon Fund). Please fax or mail to the AMS your Donation Form and inform the AMS that the funds are being transferred electronically, the bank name and the date of the transfer.
